Speaker Bios:
Jason Sasser, PTA, CWT, CSST, COQS

Jason Sasser has 17 years of clinical and operational experience as a home health field therapist and former Vice President of Operations with Wiregrass Therapy Solutions, LLC / CareSouth Home Health. As a Certified OASIS Quality Specialist, Certified Senior Strength Trainer, Certified Weight Trainer, and therapist, Jason brings a plethora of resources and tools to achieve operational success from foundation up. 

Jason has structured, developed, and implemented 20 PDGM clinical pathways founded on evidence-based research and pathway utilization that showcases outcomes for home health agency clinical best practices as well as financial viability.

Derek Michael PT, MPT 

Derek is a focused healthcare professional with over 10 years of healthcare industry experience in business development, clinical and administrative positions, with progressive leadership roles. Derek has been directly responsible for development and implementation of therapists’ day-to-day including evaluating value propositions and finding solutions to home health agencies. 

Derek’s work in the Oklahoma and Texas markets helped build and grow practices and facilitates systems to ensure care for patients across the continuum. As an OASIS certified specialist, Derek looks at the clinical needs of programs to fit the success and meaningful goals of the patient.

Diana (Dee) Kornetti PT, MAm HCS-D, HCS-C

Dee, a physical therapist for 35 years, is a past administrator and co-owner of a Medicare-certified home health agency. Dee now provides training and education to home health industry providers as Owner/Founder of a consulting business, Kornetti & Krafft Health Care Solutions, with her business partners Cindy Krafft and Sherry Teague.

Dee is nationally recognized as a speaker in the areas of home care, standardized tests and measures in the field of physical therapy, therapy training and staff development, including OASIS, coding, and documentation, in the home health arena. Dee is the current President of the American Physical Therapy Association’s Home Health Section and serves on the APTA’s national Post-Acute Work Group.  She serves as the President of the Association of Homecare Coding and Compliance, and a member of the Association of Home Care Coders Advisory Board and Panel of Experts.  She has served as a content expert for standard setting for Decision Health’s Board of Medical Specialty Coding (BSMC) home care coding (HCS-D) and OASIS (HCS-O) credentialed exams. She holds current credentials in Home Health Coding (HCS-D) and Compliance (HCS-C) from this trade association.  Dee is also on Medbridge’s Advisory Board for development of educational content on its  home health platform, and has authored several courses related to OASIS, Conditions of Participation (CoPs) and therapy.

Dee is a published researcher. on the Berg Balance Scale, and has co-authored APTA’s Home Health Section resources related to OASIS, goal writing and defensible documentation for the practicing therapist.  Dee has contributed chapter updates to the Handbook of Home Health Care Administration 6th edition, and co-authored a book, The Post-Acute Care Guide to Maintenance Therapy published in 2015, along with an update in 2020 titled, The Guide to Delivery of Home-Based Maintenance Therapy that includes a companion electronic workbook.

Dee received her B.S. in Physical Therapy from Boston University’s Sargent College of Allied Health Professions, and her M.A. from Rider University in Lawrenceville, NJ. Her clinical focus has been in the area of gerontology and neurological disease rehabilitation. 

Jason R. Falvey DPT, PhD

Jason Falvey is a PhD trained clinician-scientist who studies post-acute care for older adults. He has been a physical therapist for 10 years, working mostly in the home health care setting,  and also holds a board certification in geriatric physical therapy. Jason completed his PhD at the University of Colorado, Anschutz Medical Campus and his post-doctoral fellowship in the Yale School of Medicine. He is now an Assistant Professor in the Department of Physical Therapy and Rehabilitation Science and in the Department of Epidemiology and Public Health at the University of Maryland School of Medicine. 

He has authored or co-authored 28 peer-reviewed publications and has been an invited speaker at multiple national conferences.  His research evaluates utilization and impact of post-acute rehabilitation on functional recovery, community reintegration, and symptom burden. Jason’s research additionally extends to assessment of how environmental factors influence successful aging in place for vulnerable older adult populations.
